Nalbuphine Hydrochloride 1.0 g; Excipients q.s.f 100.00 mL.
What is Nalbufine™ 10 Richmond?
Nalbuphine Hydrochloride is a synthetic phenanthrene chemically related with naloxone (narcotic antagonist) and oxymorphone (analgesic morphine agonist). Nalbufine™ 10 Richmond is used as narcotic analgesic for somatic and / or visceral origin pain control. It is used as an enhancer of the sedative effect of major (phenothiazine) and minor (benzodiazepines) tranquilizers.
It can also be combined with NSAIDs analgesics (nonsteroidal antiinflammatory drugs), and can be used in conjunction with anesthetic and hypnotic agents (propofol, sodium thiopental, ketamine and halothane).
Bioactivity
Nalbufine™ 10 Richmond is an agonist / antagonist action opioid analgesic, equipotent to morphine (1/1). Its onset of action is fast and its half-life is longer when applied subcutaneously.
Nalbufine has a strong agonistic effect on kappa receptors and strong antagonistic effect on mu receptors. Its agonistic effect on sigma receptors is minimum and it has no effect on epsilon and delta receptors.
Indications for Use
General:
Nalbufine™ 10 Richmond is indicated for mild to moderate degree pain control.
As part of the preanesthetic medication:
Nalbufine™ 10 Richmond may be combined with major and minor tranquilizers as well as together with analgesics used in preanesthetic medication enhancing its sedative effect. The doses of major tranquilizers (acepromazine) may be reduced up to 70 % depending on the case (particularly in large-sized dogs and / or debilitated).
In sport horses it may be combined with romifidine without modifying the cardiovascular effects produced by it.
Intraoperative:
The use of Nalbufine™ 10 Richmond is indicated when for some reason it is required to strengthen the analgesia during a surgical procedure without deepening the anesthetic level. (The acting professional is suggested to pay particular attention to the monitoring of the respiratory system during the next 10 minutes after application of nalbuphine in search of respiratory depression).
Minor surgery:
Nalbufine™ 10 Richmond can be combined with a hypnotic agent (Midazolam, propofol, thiopental) or a dissociative anesthetic (ketamine) to perform minor surgery or invasive diagnostic procedures. These protocols can be complemented with the use of local anesthetics (bupivacaine, lidocaine) leaving the use of them at the discretion of the acting veterinarian.
Postoperative analgesia:
The use of Nalbuphine Hydrochloride is indicated for the control of postoperative pain of somatic and / or visceral origin.
Traumatized Patients and Critic Patients:
Nalbufine™ 10 Richmond may be used in shock of any type and origin, patients with heart failure of valvular and / or myocardial origin.
Patients with Gastric Dilatation-Volvulus Syndrome (GDV) show a significant improvement when they are supplied with Nalbufine™ 10 Richmond.
The use of Nalbufine™ 10 Richmond is indicated in trauma patients with single fractures and / or luxations.
NalbufineTM 10 Richmond is useful to control pain caused by burns of any kind and degree. In patients with drowning syndrome, nalbuphine hydrochloride can magnify it.
NalbufineTM 10 Richmond is indicated to control pain from eye trauma.
In horses it is very useful for controlling pain from colics, alone or combined with flunixin meglumine.
It is also useful to control the pain caused by Degenerative Joint Disease of horses and musculoskeletal pain in general.
As morphine antagonist:
NalbufineTM 10 Richmond is complementarily indicated to partially reverse the effects of pure morphine agonists (morphine, fentanyl, etc) preserving its analgesic action without the side effects of naloxone (morphine antagonist).
The reversal of the effects of morphine agonists does not imply any risk of renarcosis.
Dosage
Dogs: 0.5 to 2 mg / kg SC, IM, IV.
Cats: 1.5 to 3 mg / kg IV.
Sport Horses: 0.3 mg / kg IV.
Interactions
Nalbuphine can interfere with the prokinetic action of metoclopramide.
The simultaneous use of hydroxyzine and nalbuphine may enhance the analgesic and depressants effects of the CNS.
When used together with anesthetics or hypnotics their doses should be reduced.
Contraindications
The use of nalbuphine is contraindicated in patients with known hypersensitivity to the drug.
Contraindicated in patients with cranioencephalic trauma and / or rachidian trauma due to local vasoconstriction and aggravate the condition.
In patients with severe respiratory depression.
Asthmatic cats.
Patients with severe liver or renal dysfunction.
Studies on fertility and reproductive capacity in rats revealed no influence of nalbuphine on these functions.
Teratogenicity studies in rats did not show any effect. However it is suggested to be used with extreme caution in pregnant females until more scientific information is available.
Precautions
Verify if the product inviolability system and its storage conditions are appropriate prior to its use.
Suggested doses and warnings in all cases are to be determined by the veterinarian.
Store between 15ºC and 30ºC, protected from direct sunlight, in a dry and hygienic place.
The product must be administered alone in the syringe; it should not be mixed with any other compound.
Use Restrictions
Do not use in horses intended for human consumption.
Poisoning and Overdose
In case of an iatrogenic overdose is recommended: hemodialysis, peritoneal dialysis and appropriate symptomatic treatment according to the criteria of the veterinarian.
Suggested antidote:
Dogs: Naloxone 0.04 mg / kg IM, IV, SC
Cats: Naloxone 0.05 to 0.1 mg / kg IV
Sport Horses: Naloxone 0.01 to 0.022 mg / kg IV
